Fingerprint Clearance Card

All education students must have a class 1 fingerprint card prior to their student teaching semester. You may pick up a fingerprint application packet from the Office of Professional Field Experience during business hours. Your local police station will imprint fingers. Please call your local police station for further details on fingerprinting. After your fingerprints have been rolled, read and follow the application instructions included in the fingerprint packet. If you have any questions regarding the application, or any concerns related to obtaining a clearance card, call 602-223-2279 (Arizona Department of Public Safety Applicant Clearance Card Team).

When you have received your clearance card, make a copy of the card and bring the copy to the Academic Advising office in the College of Teacher Education and Leadership. You will not be given a student teaching request for placement form until the Office of Professional Field Experience has received a copy of your class one or two clearance card. Your class one or two clearance card will also be required for state certification procedures.
